Chronic conditions are overwhelming our healthcare system

Among developed nations, the US has the highest chronic disease burden 1

Our healthcare system was not set up to support chronic conditions. 2 A number of factors contribute to this:
While a senior health crisis is growing due to the lack of support of adults in their golden age

of those 65+ are isolated, which is the health equivalent of smoking 15 cigarettes a day 3,4
Related needs
Greater social interactions, environmental awareness and cognitive engagement

is the average time someone 65+ spends with a clinician annually 5
Related needs
More touchpoints, richer engagement and information that contextualizes their health
We created Clarence Health to elevate health outcomes at scale and aid the caregivers and institutions that support these populations
Clarence Health was created by Andy Lozier after he experienced firsthand the challenges of managing his father's chronic conditions, as well as his own, within the fragmented U.S. health system.
Prior to founding Clarence, Andy created and led the New Ventures & Business Incubation function at OhioHealth, a $7 billion health system. In that role, his team evaluated disruptive technologies, managed the system's venture partnerships and investments, and helped build innovation capabilities across the organization.
Jim practiced medicine as an intensivist and pulmonologist prior to becoming a physician leader & administrator.
He is currently Chief Medical Officer at AndHealth, a specialty care provider. Previously, Jim led population health operations and served as head of quality for a sixteen-hospital regional health system in the Midwest.
